Urban Lazer Tag Waiver Form

For all participants.....you must download, print, read and sign Urban Lazer Tag Waiver and Release of Liability Form before participation. For anyone under the age of 18 a parent and/or legal guardian will need to sign waiver form in order for you to participate. If waiver form/s not signed and received by Urban Lazer Tag you CANNOT participate in any laser tag event hosted by Urban Lazer Tag until doing so. Waiver forms need to be signed and turned in at start of any event to help accommodate all guests. We will collect these at the start of all events for those who plan to participate in laser tag/gelball game play.
